We need a Php/Mysql or even simple html page, with a certain form that (inculudes informations such as: Name, address, phone, email, current date etc...) that will be able to print each variable into a specific zone/area of a PDF or Jpeg file. In the Php/Html, beside each field of the form, i'd like to be able to enter the coordinates where this data will have to be printed (and if there are more than one page in the PDF/Jpeg then i should be able to select which page number). When i click "Generate PDF" at the bottom of the page it will generate and start the download of the PDF document that will have all the data printed at the right place.
