I need a simple upload form created that allows someone to upload up to 10 images to a directory on my server. there will be 2 additional boxes for them to enter their name and email. for each image, they will be able to select one of 10 categories from a pulldown menu. The file name of the image you save on my server will be a combination of the category, their name and the image name. This will need to appear on my drupal website. I will show you example of what I need done. I assume this will only be one to two hours work and the bid should reflect that - e.g. - $30-$40.
