Create a questionarre, that has many options and paths
I need somebody to create a form that, through jQuery, or something like that, will create a survey for users to answer. All I need is a template, and from there I could fill it in. It would need to email me the results. This is what it would look like: (Depending on what option you select, you get different questions) What kind of website do you want? Website - What color - How many pages - do you need a domain name? - How quickly does it need to be done - This will cost about x dollars (depending on how many pages they chose, and how fast it needs to be done) eCommerce Site - How many products - what is your time frame - do you have a domain name? - what languages will this be in - what type of site is it - this will be $X (depending on what they choose Blog - etc. Forum - etc. This would be very simple for some people, so my budget is under $50. I just need a template for this. Lowest bidder with examples will probably win.
