PSD to HTML Conversion and CSS

I have a one (1) page website designed in Photoshop. It has two (2) forms on it and a video holder. I have already slice the design. I need someone to do the markup and styling for me. If you're interested, let me know your rates. It's quite basic and I think the video holder and the forms will be the only problem. You need to know how to style these elements before you accept this project. I have attached a PNG image file of what the PSD looks like so you can guesstimate the number of hours you can take to complete the job. Give me an hour range for completion if possible.
