I have a 1 page PDF form that needs to be made into an app to run in both windows and Mac environments EMS APP 1. As well as for use in Iphones tablets, androids etc. 2. Ease of use (to fill in fields) on mobile devices 3. Fill automation (e.g., names of hospitals once added should be saved system wide, yes/no questions only one option) 4. Ability to print Email or E fax the same look as the original document 5. Upon completion of form, it should convert the form to pdf and automatically be (a) saved (b) sent to two default addresses (c) be sent to a 3rd party from a pull down list {to be added later] 6. Once completed no further editing should be allowed 7. It must be user friendly, simple and clean.
