I want a html webform whereby the user logs into a website and is presented with a simple html5 mobile form. The form has fields that I will specify that can be text or number - some will be manditory. The form also has a button that will allow a user to capture an image using the iphones camera The form will also gather the GPS location of the user. At the end of the form, the user can upload the form and all details to a simple SQL server. The scope of the project includes creating a PHP dashboard where the administrator can view all forms that have been uploaded. A webhost will be given and the PHP must be compatible. The scope should also include a graphical interface of the web page. 1 month of free technical support should also be included and the final build of the form should work without bugs.
