There is a need of an online interface where all the information is taken by the user and a Word report is generated after submitting the information. It is also required to save all the information in a database (SQL). To summarize: 1) User fills the information in an Online form 2) Information is saved in SQL 3) MS Word file is generated out of the stored data The format of the Word file is standard and will not change for each user. The Word file will be structured and include colors and formatting. A sample word file is attached with the request. This is a sample to provide an idea of the formatting and the fields.
