I'm looking for a simple app for iPad that would allow patients to come into the office, enter some information and that information would be emailed to a few email addresses in the back office, text messaged (via iMessage and SMS) and posted to a online spreadsheet (like GoogleDocs) to know that the patient has arrived. Fields must be able to be edited, deleted and added.
