This bid would be sent automatically for those contractors that have bought into the "platinum" package. The bid request form would include; Category (required), automatically generated bid number, First Name (required), Last Name (Optional), Email (required), Phone (Optional), When project is to be started (Check box with "Within 30 days", "Within 90 days", "Within 6 months", "Within a year", "Unsure" (required), Text box for "Comments".
