Populate PDF form from Access database
This should be pretty simple. I have an access database that has 95% of the information required for a PDF form that I have to generate for our non-profit clinic that is required by insurance companies (we are an autism treatment center). The database has all of the client info, their various insurance companies (each company has their own codes for our services), and the information we need for insurance companies to fill out the forms. I would like to have a button in Access that will take the attached form, and populate it from the appropriate fields in the db. This would require a dialog box that would pop up when you click on the button that would ask which child we need the form for. From there, it would have a dialog that would allow us to use a dropdown of up to 15 items for that child's insurance company's codes (they are in a table in the db that contains the codes for each insurance company, and each child has a field that tells which insurance company is theirs), and then we will need the dialog to accept us entering how many hours at each code. And dates. We also need to enter one alphanumeric string that is the insurance company's authorization. We'd then like all of the information for each one saved in a table for each child so that we can recreate any past 1500 form that we have previously generated. Here is an example of the 1500 form that insurance companies require of us. The fields are already defined, so that work has been done. Once I have bidders, I will send our database. I have to empty all the tables because of HIPA laws, and haven't had time to do that.
