I require an online application form creating with back end database and a case tracking system. The application form will have approximately 10 pages and each page will have about 10 fields on it. The form will be made up of standard form fields and is to be used by financial advisers for them to submit applications. The form will need a feature to allow the user to save progress When the user has completed the form, the results need to be submitted to a database. There are a couple of simple logic conditions required which will allow the user to enter details of current pension products they own and if they own more than 1 pension allow the user to enter the details for more pensions. The database also needs to have a feature to add users who can login to access the database/stored form results. There needs to be an admin user who can view all form submissions. Users who submit forms will also need a login so they can view all the applications they have submitted. I need an option to allow a user to print a copy of the application form. Finally, I need a feature which will allow case tracking notes to be entered on the system so the admin and the user can both leave comments on the progress of an application. The comments need to be date/time stamped and no option to delete comments as they are required for auditing purposes. The project is urgent and I need to get on with this as soon as possible.
