I need a custom app for my martial arts school which will be used as a consultation form instead of having people filling out pieces of paper and then entering it into infusionsoft. I have attatched screenshots of my friend's application. It must be able to work on both ipad and android. I will hand tablet to prospect and they will fill out all their info and take picture of themselves with the tablets camera and it will attatch to their record in infusionsoft.
