Outreach Worker Mitchell Community Health Service Mitchell Community Health Outreach Worker (up to 1.0 EFT) PHaMs Mitchell Community Health Service (MCHS) is a vibrant multi-disciplinary organisation providing extensive health and social support services to communities within the Mitchell and Murrindindi Shires that adjoin northern metropolitan Melbourne. Due to a recent expansion in service provision, MCHS currently has two vacancies for Outreach Workers to join their Personal Helpers and Mentors (PHaMs) team based in Broadford. The PHaMs program uses a strength based recovery approach to provide services to people whose ability to manage their daily activities is impacted by severe functional limitation resulting from mental illness. The Outreach Worker role is a specialist role that is critical to the success of the PHaMs program, and provided throughout the Hume - Goulburn Valley region. Outreach Workers have a range of skills and qualifications, as this role encompasses the provision of appropriate supports / linkages and empathy to assist in engaging and encouraging participants with a mental illness through their own recovery journey. The successful applicants will have experience and knowledge of working with people with a mental illness from a case management perspective. Ideally, the applicant will also have experience with CALD, Humanitarian Entrants, people either homeless or at risk of being homeless and young people leaving out of home care. A Certificate IV in Mental Health and at least 12 months experience in Case Management is required. The positions are up to 1.0 EFT with work hours between 8.30 am and 5.00 pm although some flexibility on days worked may be required to meet the needs of the service. Appointment to a position will be on a fixed term basis subject to the availability of funds. Currently it is funded to 30 June 2014.
