We need a simple website with the following functionality: 1. A text form to gather client information. - The client will enter this information themselves 2. This website requires that the client can upload photos together with their personal details 3. When the client clicks 'submit', the information and the photos are automatically emailed to me in a PDF template - if it is easier, a Word template is OK too. 4. The back-end should be able to record every submission for database purposes. 5. EACH SUBMISSION SHOULD HAVE A UNIQUE ID The site will only have 5 pages: 1. Home (Submission page) 2. FAQ 3.
