I'm currently running a Wordpress website for my parking business. That website is in turn using a plugin entitled 'Custom Contact Forms'. I need someone to make a few revisions to this plugin, which I just can't seem to figure: - Firstly, I need a checkbox at the bottom of the form where it asks the user 'You agree to the terms and conditions of this website'. This box must be checked if the user wants to submit the form. The text of 'terms and conditions' must include a hyperlink to the Terms & Conditions page. - Lastly, the bookings requests provide me with dates in the following format MM/DD/YYYY.
