I have a 3 page word document (essentially a table) . I wish to have that document converted to a form where users can fill in the fields. The final form is then stored in a mysql database and a pdf of the completed form is emailed to a nominated address. Minimum Features required a) mysql database backend , linux server host b) completed form emailed in pdf c) database stored and able to be searched/retrieved at any time afterwards d) able to change destination email address easily e) ability to modify form text easily to provide for change Note that the attached document is a draft and not the final document planned for conversion.
