I am a small multidisciplinary clinic in California looking to develop survey my patients. The practice is focused on bariatrics (weight loss) and involves a team of physicians, RD's, behaviorists and exercise physiologists. Patients are with us in several phases, each of which needs to be surveyed. Standard outpatient clinic surveys do not fit our model. The purpose is for quality improvement. I am looking for assistance in developing the surveys well as determining the best approach for implementation (web, paper, etc).
