Permanent fulltime RN needed to work as part of the Marrickville Mental Health Team providing case management based at Marrickville Health Centre. Previous applicants need not apply. To provide comprehensive community case management services for residents of the Marrickville Catchment Area who have a mental illness/disorder. The Marrickville CHC provides services for people with a mental illness living in the community who require mental health intervention. The team provides evidence-based, active care coordination taking a whole of life approach and ensuring continuity of care to individuals and their families and carers. The clinical case manager (or care coordinator) provides continuing treatment and coordinated care in the community, particularly where multiple support agencies are involved in the care plan. Functions of the role: Assessment; care planning and review (including individual care plans, relapse prevention plans and advance care directives); working in partnership with other government agencies and families and carers; and provision of short term, stepped or ongoing treatment and care coordination including pharmacological and psychotherapy interventions. The Marrickville Mental Health Teams provide an integrated model of care and this requires a level of integration and sharing or roles across the clinical streams.
