Duration: 3 months Location: Savannah, GA/Jasper, GA/Cedartown, GA/Dalton, GA/Vidalia, GA Description/ Required Skills: - Performs patient evaluations and provides therapeutic services according to physician’s orders. - Responsible to: Director of Operations / Clinical Manager Directly Supervises and Evaluates Physical Therapy Assistant and Home Health Aides as assigned. State Specific Description -Wisconsin State Specific: “Therapeutic service” means physical, occupational, speech or other therapy, medical social services, home health aide service, or any other medically oriented service except skilled nursing care. State Specific The qualified therapist duties include: - Providing treatment as ordered by the attending physician, dentist or podiatrist. Physical Therapist Page 3 of 7 July 2010 - Assisting the physician in evaluating level of function. - Assisting in developing and updating the plan of treatment. - Observing, recording and reporting information on the patient's condition to the attending physician and in the patient's health record. - Advising, consulting and, when appropriate, instructing family and other agency personnel, in patient's therapy program. - Teaching and supervising other health personnel when appropriate. - Evaluating the home environment and making appropriate recommendation. - Participating in in-service education programs. Virginia State Specific: - Provides skilled physical therapy in accordance with the physician’s plan of care. Wisconsin State Specific: - HFS 133.15 Therapy Services: Provision of Services: - Physical therapy services provided directly by the home health agency or arranged for under s. HFS 133.19, shall be given in accordance with the plan of care developed under s. HFS 133.20. - Individuals providing these services shall perform the duties under s. HFS 133.14 (2) (a), (c), (f), (h) and (i). 2. Physical therapy. - If offered, physical therapy shall be provided by a physical therapist or by qualified therapy assistance under the supervision of a qualified physical therapist. - The physical therapy is licensed and assist in general practice of physical therapy under the general or direct supervision of a physical therapist who: Assumes primary responsibility of care provided by the PTA Has direct face-to-face contact with the physical therapist at least every 14 days Is available in interim via telecommunication between direct contact while PTA is providing patient care Provides written and oral communication describing supervision and management of the PTA providing patient care Delegates the appropriate treatment plan and program conducive to PTA competency and training. - Provides on-site assessment, reevaluation, and adjustments to treatment at least once a calendar month or every tenth treatment day (whichever is sooner) of all patients. - Coordinates discharge plan decisions and final assessments with the PTA Provides general supervision of no more than 2 full time equivalent PTA’s to ensure all patients receive appropriate care. -Home visits. - Periodic office meetings. - Current and unencumbered license to practice physical therapy specific to that state the employee is assigned to work by the Company. - One year (1) of physical therapy experience preferred. - Current CPR certification. - Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs." "State Specific-qualifications. Florida State Specific: - The physical therapist shall be currently licensed in the state, pursuant to Chapter 485, F.S., with at least 1 year experience in physical therapy. - Physical Therapist Page 6 of 7 July 2010 Nevada State Specific: - NAC 449.788 Services to patients. - Services must be supplied only by qualified personnel and under the supervision of a physician licensed to practice in this State. - Qualifications include licensure, registration, certification or their equivalent, as required by state or federal law, for…(d) The physical therapist must be registered in this State. Wisconsin State Specific: - HFS 133.02 Definitions: “Physical therapist” means person licensed to practice physical therapy under ch. 448, Stats." Education: - Successful completion of a physical therapy program at a school of physical therapy approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of American Medical Association or the committee on Allied Health Education and Accreditation of the American Medical Association.
