Organization name Hamon Funeral Service Description of the organization and its target audience People interested in pre planning their funeral service, for them to fill out either on their own or with the help of the funeral director. The font on the brochure should be simple & easily read by elderly people. Brochure to design Half-fold (4 sides) Legal (11" x 17") Requirements - front of brochure Text, space for picture, form to be filled out (death certificate info). I attached a blown up copy of the death certificate so the form can be more easily read. Requirements - body of brochure On the first inside page (left) the questions in order are: Poem, verse, saying, Whis is this special, Obit/Bio, Where & when were you born, When did you profess & through who, Who did you marry & when, What did you do for work, Special/Remarkable story or facts, Family Members. On the second inside page (right) the page is headed Service Information. The questions in order are: Who would you like to have your service & why, Who would you like to sing at the sermon & why, Who would you like to play what instruments & why, What songs would you like played & why, Who would you like to have as pall bearers if available (please list more than 6, should some not be available) any special notes as to why some were chosen, Which cemetary, Casket and/or urn, Favorite flowers and or colors? Requirements - back of brochure Picture/Song/Vers/Poem Why is this special to you? (then space for another picture. Below that Budget followed by: Funeral Service Charges, Hall rental, Catering, Casket, Cemetary, Folders/Memorial Book, Flowers, Urn/Marker. Please also include: Company name, address & phone number: 16009 Dayton Ave N Seattle, WA 98133 206-947-8245 (maybe at the bottom of the page?) Style/theme ideas for the brochure Possilby a deep blue outline or maybe have the lines to be filled out in blue. Which ever looks best to incorporate blue somehow. What to avoid No, just a note though; the 11x17 brochure will be folded in half once horizontally and then in half vertically. Attached is a rough outline of what we are looking for. We would like the option of printing the brochure ourselves or adding it to a blog so customers can print it themselves, fill it out and mail it in. Thank you!
