Independent Contractor Opportunity to Participate in a Texas Statewide Special Project     BECOME A PART OF AN EXCITING STATEWIDE PROJECT IN GEORGETOWN & PARIS:   Money Follows the Person Quality of Life Survey (MFP QoL) Registered Nurse, Licensed Vocational Nurse or Social Worker Independent Contractor Opportunity   NACES Plus Foundation, Inc. Is seeking interviewers for a project that began in 2008 and will end in 2016.  This is part time work in conjunction with the Texas Department of Aging and Disability (DADS).  This is a nationwide effort to remove barriers to community living for people of all ages with disabilities and long-term illnesses.  These are Medicaid beneficiaries who want to live in the community and could do so if they had adequate support.  This project is to interview Medicaid beneficiaries who are transitioning from institutional facilities into the community where they can be served.   Individuals will be interviewed pre-transition, again at 11 months and 24 months to assess their quality of care and quality of life.    Minimum qualifications: A current TX license in area of practice in good standing, with one (1) year of verifiable experience working in a community setting (includes small group homes, foster care, community clinics, Hospice and home health experience) with people with cognitive disabilities (e.g. mental retardation, Alzheimer’s disease, dementia, autism, pervasive developmental disability).    Work hours must be flexible to meet the needs of the person being interviewed.  An excellent opportunity for retirees.     To request further information, please email your resume to nacesrecruiter@texasnurses.org or fax to (877) 696-2237.     Texas Nurses Association NACES Plus Foundation, Inc. 7600 Burnet Road Suite 440   Austin Texas 78757 www.nacesplus.org
