I need a custom app for my martial arts school which will be used as a consultation form instead of having people filling out pieces of paper and then entering it into infusionsoft and also zenplanner.com. I have attatched a file containing and explanation and also some examples. I will hand an ipad or tablet to a prospect and they will fill out all their info and take picture of themselves with the tablets camera and it will attatch to their record in infusionsoft and also zenplanner with all data going into both of those programs in the appropriate fields.
